SILICONOMIC AND BIOPOLYMERIC EXTIPATION – Lip reconstruction after foreign body reaction

This type of consultation is becoming more and more frequent in our clinical practice.

We are usually consulted by girls who have had a substance injected into their lips 10 – 15 years ago. Often it takes years for the problem to appear, other times the problem appears early: hardness is generated, a kind of small balls on the lips that distort the lip contour.

This is usually because the substance injected in the past is a non-absorbable substance (such as kerosene, silicone, biopolymers…) and over the years the body reacts against them. As it is not able to eliminate this substance, metabolize it and make it disappear, what it does is to encapsulate it in order to isolate it within the tissues. And it is for this reason that these hardnesses appear, sometimes real balls or benign tumors.

The extreme or best known case is when this deformity attacks the sides of the lips and the commissure, with the patient acquiring a “duck-billed” deformity, where the lip has great volume at the lateral level and appears totally unbalanced. Other times, the lip elongates and droops, giving an aged appearance.

How are these lips reconstructed?

The overgrown tissue can be removed and the lip can be restored to harmony and shape. It is a simple procedure, it is performed in consultation with local anesthesia and resorbable stitches are placed, they fall off by themselves within a week. The procedure is quick and painless, and postoperatively some lip swelling may occur, which will be treated with analgesics. The scar is located within the oral mucosa and is imperceptible one week after the procedure.

The purpose of this procedure is to remove this product and the inflammatory reaction that surrounds it, and on the other hand, to restore the shape and harmony of the lip. Subsequently, new infiltrations can be considered, but in this case with hyaluronic acid, which is a safe filler and does not generate this type of inflammation.

Another accessory procedure is the so-called
lip lift
which shortens the lip and gives it a more youthful appearance. It is an ancillary procedure that also improves the appearance of lips that have biopolymer deformities.
